Effective for any Phase I Environmental Site Assessment (Phase I ESA) performed after September 15, 2023, updated the:

required data elements in the Environmental Due Diligence Requirements (Form 4251); and
Compliance Agreement for Radon O&M Plan (Form 6420.Radon).

Primary Changes

The Environmental Professional must deliver the required radon- and property-related data elements specified in the updated Form 4251.
Updated Form 6420.Radon to require compliance with an O&M Plan for radon remediation prepared either before or after the Mortgage Loan Origination Date.

Superseded Publication

This publication supersedes Notification 23-13: Form 4251.
